“Initially I received  letter from Xiaohai on Feb, 2006. I was visiting in overseas; I immediately gave him the answer when I came back . It seemed he worried about his disease very much.” Dr Xiaodi Han introduced.

Xiaohai was exactly a 33 years old patient with long medical history when he sent these enquiries on Daifu MD.com. Xiaohai is his ID name..Since Xiaohai has got partial seizure for 13 years and his symptom was very mild and one kind of affective illusion. This seizure always lasted 30 seconds and happened every half month with quick recovery. He didn’t pay attention to this until generalized tonic-clonic seizure happened on him six years ago . He started his medication treatment : Tegretol 0.2 t.i.d initially, then generalized tonic-clonic seizure happened 1-2 times per year .In recent three years Topamax was prescribed 100mg B.i.d, the generalized tonic-clonic seizure disappeared. However, the complex seizure were more serious. The seizure persisted 1-2 minutes but it took at least 4-5min from somnolence to awareness , and average 7 days per time. MRI Indicated 6^th ventriculus formed. 24 hour EEG indicated spindly wave formed in sleeping. 7 spike waves and spike –wave complex could be seen during whole monitor process. No clinical attack  Indicates :Epileptiform activity  Shanghai Gamma knife hospital PET indicates: Blood stream to left hippocampus and amygdaloid body reduced.

Xiaohai was very anxious to his disease. He wanted to know whether to continue his conservative treatment or operation immediately. Also whether conservative treatment cause injuries to his brain . Whether the operation would cure the diseases and how possible the operation damaged the speech center. Meanwhile he cared about whether the operation cutting affected his outlook.     

After several enquiries, Xiaohai decided to come to Beijing to have a treatment in the end of June. Hospital arranged the consultant from Epilepsy Center. After the examination of MRI, CT and EEG, combined with the clinical symptom and sign, Neurologist and Neurosurgeon had an extensive discussion and made a diagnosis of secondary epilepsy .That possible originate from left temperal dysembryoplastic neuroepithelial tumor (DNT).

This kind of tumor patients with epilepsy that medication can not control usually had their clinical attack before 20 years old. The most often lesion located in frontal lobes and temporal lobes. Rare lesion located in parietal lobes. especially occipital lobes。Surgery can be great helpful, the tumor can be cured even under partial tumorectomy.  

The consultant center finally suggested the operation was a better option for Xiaohai. After his agreement, neurosurgeon had a left temporal lobe craniotomy and had the epilepsy focus moved. The pathologic biopsy supports DNT.

During the hospitalization period, Xiaohai had lots of enquiries about his postoperative condition and presented obvious depression and anxiety. In order to relieve his doubt, Dr han and other neurosurgeon explained the operation process and pathologic biopsy outcome, also some research and development on literature related with this disease are explained.

Usually, the long term epilepsy patients have heavy psychological burden. A doctor, whether a surgeon or a physician, should pay attention to patient’s psychological status. Xiaohai has never had epilepsy attack since operation. His medication dose is reduced. He starts a new life like normal people without anxiety towards his future.
